More space for transitional care – Lelystad

Woonzorg Flevoland opens the expansion of the “bridging section” at the Hanzeborg residential care site in Lelystad. This will free up an additional 12 places for people waiting for a place in a nursing home, but living at home is no longer possible. There is a great need for this in the region.

Faster flow
The expansion will allow for a quicker move into a home on a residential care site. This way more people are helped. This was stated by Care Director Pascal Quadflieg.

Residents of the “bridging section” can wait in the section in an apartment in the nursing home in Woonzorg Flevoland. But they can also move to a home from another regional care provider.

Initially 8 places
Initially, the “bridge section” had 8 seats, but with the expansion, the number has now reached 20 seats. One of those rooms is the crisis point. This is an emergency room and is intended for people who need acute care.

It is also about the fact that care at home is not enough. For example, the organization may be called by a GP and informed that someone will be coming to the crisis room within one hour.

This means you have to act quickly, prepare the room and take good care of the client and any relatives who come with you. For clients, living in a “transitional section” means the end of independent living.

The question for the team is to manage the transition as best as possible. This means, among other things, that during the personal interview there is room for the client and any family member to ask questions.

You must also determine what care and support will look like in your home situation. Then – if possible – this can be continued in the “Bridging Section”. The client’s capabilities, the role of the informal carer and the additional role of the care team are also taken into account.

There is a new concept in “Bridging Department”, where the customer network plays a key role. This includes partners, children, friends and acquaintances. In this way, professional care complements informal care.

This means, among other things, that caregivers and nurses are responsible for physical care and support. Finally, healthcare technology can be used, such as a medication dispenser. This is the device that dispenses the medication.
