From now on, projects implementing a permanent oncology focal point (VAP) in practice outside hospital walls can apply for funding for implementation and evaluation. The implementation round is part of the oncology project for a permanent contact point (VAP) outside the hospital walls. Approximately six project institutions will be given the opportunity (to further) implement and evaluate oncology VAP (local/regional).
VIOZ Foundation and KWF intend to provide funds for this purpose. We also warmly invite initiatives and “good examples” that have already shared information with the project team during the first inventory round to register for the application round for implementation projects in order to qualify for funding.

A fixed point of contact for each patient
Cancer patients find a constant point of contact very important in the hospital, but also outside it. In order to organize continuity of care and support outside the hospital walls, the 3-year Oncology Outpatient Permanent Contact Point (VAP) project was initiated on May 1, 2023. A project from Cancer Survivor Care Task ForceOf which IKNL is a part, as part of the National Action Plan on Cancer and Life. Through this project, the Cancer Survivorship Care Team wants to provide a permanent oncology contact point (VAP Oncology) outside the hospital walls for every patient living with or after cancer who needs one. The role of VAP oncology outside the hospital walls can be achieved in different ways. This includes registering this role within existing and structured working agreements between organizations in formal and informal care and/or e/online/hybrid healthcare applications.
