The mouth mask may be gone in most places as of October 1, but the PLF form remains mandatory for the time being for those who have been abroad for more than 48 hours and have returned to our country. This form is free, but scammers are now trying to sell completed fake documents for a fee.

If you return to Belgium from abroad after a stay of more than 48 hours, you must fill out a PLF form that allows you to be identified and contacted, especially if you have remained in a danger zone.
This form is only downloadable via https://travel.info-coronavirus.be. There is no fee, the data collected is confidential and will be destroyed within 28 days. “Do not fill out any other document requesting personal information and payment,” the Federal Public Health Service warns.
